Student Health Advocates
Overview
Connect2Health health advocates meet individually with patients to understand their needs and priorities. They collaborate with patients to create a personalized "Resource Prescription" using the Connect2Health Resource Database.
Connect2Health is dedicated to empowering individuals to utilize community resources for multi-dimensional health. For some patients, having a simple list is sufficient, while others may require more mentorship as they navigate the complex resource landscape. Connect2Health health advocates spend as much time as necessary to help patients access these resources, assisting with applications, eligibility calls, and transportation options.
In relevant cases, Connect2Health health advocates also conduct follow-up calls and visits to check if patients have successfully utilized their Resource Prescription. If patients haven't followed through, Connect2Health Mentors provide encouragement and additional support. When patients approach organizations, advocates track their progress and may refer them to alternative resources if needed. This ongoing support can last weeks or months, depending on the patient's needs. Through this consistent advocacy, Connect2Health aims to create ripples of change in patients' lives, ultimately leading to a wave of positive transformation in the community.
